A jelen nyilatkozat megtételével
- Kijelentem, hogy az Európai Parlament és a Tanács (EU) 2016/679 rendelete (GDRP) alapján a készült Adatvédelmi tájékoztatót elolvastam, megértettem és tudomásul vettem.
- Kifejezetten hozzájárulok, hogy a jelen dokumentum hátoldalán megjelölt személyes adataimat az Adatkezelő, az ott megjelölt adatkezelési céllal kezelje, és felhatalmazást adok az Adatkezelőnek a közölt adatok helyességének ellenőrzésére.
- Kijelentem, hogy a jelen nyilatkozat és a nyilatkozatban foglalt hozzájárulások megadása önkéntesen és a megfelelő tájékoztatás birtokában történt.

By making the present statement
- I declare that I had read, understood and acknowledged the Data Privacy Notification prepared in accordance with Regulation (EU) 2016/279 of the European Parliament and the Council (GDRP).
- I grant my express consent to that my personal data specified overleaf of the present document be managed by Data Controller for the data management purpose specified therein, and I authorize Data Controller to inspect wether the data provided are correct.
- I declare that I made the present statement and I granted the consents included in the statement voluntarily and having been properly notified.
Alulírott nyilatkozom, hogy
- Nincs olyan légúti fertőző betegségre utaló tünetem (pl. láz, köhögés, akut légzési nehézség, stb.), amely miatt mások egészségét veszélyeztetném és közösségben nem tartózkodhatnék.
- Az elmúlt 14 napban nem voltam szoros kontaktusban valószínűsített, vagy megerősített új koronavírussal fertőzött személlyel.
Hereby I confirm
- I have no symptoms of infectious respiratory tract disease (e.g. fever, cough, acute respiratory difficulty, etc.) that would endanger the health of others and prevent me from socializing and interacting in community.
- I have not been in close contact with a person who is suspected or confirmed to be infected with the new coronavirus in the last 14 days.
